Revised and new consent forms

On 19 February 2024, 14 updated consent forms and a new consent form will come into force.

 

These updates come following changes to the Code of Practice 9th edition (9.4) published in October 2023. The changes relate to loss of mental capacity and allow patients to consent to the continued storage of their eggs, sperm and embryos, without consenting to the use of their material in treatment whilst they are mentally incapacitated.

 

For further details, please see this Chair’s Letter  which includes copies of the new and updated forms and guidance to support implementation. Please note that while we are releasing copies of the new and updated forms now for information purposes, they must not be used in clinical practice until 19 February 2024.
