Revised and new consent forms |
On 19 February 2024, 14 updated consent forms and a new consent form will come into force.
These updates come following changes to the Code of Practice 9th edition (9.4) published in October 2023. The changes relate to loss of mental capacity and allow patients to consent to the continued storage of their eggs, sperm and embryos, without consenting to the use of their material in treatment whilst they are mentally incapacitated.

Please note that while we are releasing copies of the new and updated forms now for information purposes, they must not be used in clinical practice until 19 February 2024.
